The Department of Fire Rescue completes a state required patient care report for all medical calls. These reports are considered medical reports and require strict procedures to protect our patient’s privacy.

If you were a patient and you are in need of a copy of your report, you may request a copy of your own EMS report by contacted the EMS Cost Recovery Program Specialist at acfrcostrecovery@albemarle.org or by calling 434-296-5833, press 5.

EMS reports can be picked-up in person with a photo ID, can be mailed, emailed or faxed. For an EMS report being picked up, prior arrangements must be made with the EMS Cost Recovery Program Specialist. The Department is not able to guarantee a report will be provided to a walk-in without prior arrangements being made.

If one of the county’s volunteer agencies cared and/or transported you to a medical facility, you should direct your request for a copy of your patient care report to them.

There is no cost for obtaining a copy of a patient care report.

An attorney may request a copy of their client’s EMS report. This request will need to be submitted in writing and include the following information:

• Incident Date
• Incident Location
• Approximate Time of Incident
• Client’s Name
• Client's Date of Birth

A signed medical release form must be attached to the request. The request can be returned to the Department of Fire Rescue at the following address, fax, or email:
